The US release seems to be a direct port of the Italian version - the tell-tale signs being that both share the same interview featurette, as well as the fact that the US version includes the Italian audio track with English subtitles - something Blue Underground almost never do. The same master have clearly been used for both transfers, and the differences are not particularly striking. The US version has marginally brighter colours and also exhibits a very slight cropping of the image. It also looks a bit "harsher" than its Italian counterpart (see above for a shot of the edge enhancement that people keep telling me Blue Underground's DVDs don't have).
The BU release will probably be the preferred version for most people since it includes English subtitles for both the Italian audio and for the extras. Those who speak Italian and crave the best image quality will no doubt prefer to go for the Italian version, though.
